Your Responsibilities Will Include:
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Administer medications and injections accurately, including IV infusions and medication management in accordance with physician orders and safety protocols.
- Document all patient interactions thoroughly using EMR (Electronic Medical Record) systems like In My Team, maintaining compliance with HIPAA regulations and medical documentation standards.
- Collaborate with interdisciplinary teams—including physicians, therapists, and case managers—to coordinate effective treatment plans tailored to individual needs.
- Educate patients and families on health management topics such as medication administration, nutrition, wound care, and chronic disease management.
What You Bring to the Table:
- Valid RN license in the state of employment with current certification to practice nursing.
- Proven experience in diverse clinical environments such as hospital medicine, Home Health ,emergency medicine, or long-term care settings.
- Hands-on experience with EMR/EHR systems
- Competency in performing advanced nursing skills such as IV insertion, phlebotomy, ostomy care, and aseptic technique.
- Ability to work effectively with individuals across adults in acute or chronic conditions, and seniors requiring memory or dementia care.
- Excellent communication skills for triage assessments, patient education, and interdisciplinary collaboration.
- Knowledge ; experience in hospice & palliative medicine or behavioral health is also valued.
