Physical Exam: completed within the LAST 6 Months, stating that the applicant is free of all communicable diseases
Social Security Card
Covid vaccine card

Requirements:

Complete CEU & Domestic violence, OSHA, HIV/Aids, CPR, Alzheimer Certification, and a Health statement showing & free from communicable disease and negative TB test prior to application for employment.
